Many seniors are struggling with health problems that may jeopardize their ability to stay safely in the home they love. PACE of Southwest Michigan may be able to help. PACE (Program of All-Inclusive Care for the Elderly) is a unique program of coordinated care that allows older adults to remain in their home. Our team of physicians, nurses, pharmacists, social workers, and therapists surround our participants with medical and private services to help them stay strong, independent, and in the community they love.

Come join us on Thursday, August 24 at noon as Connie DiDonato, Marketing and Community Outreach Coordinator at PACE, shares more about the PACE program, some wonderful success stories, and eligibility requirements.
